Have a doctor in mind?
If your favorite doctor isn’t in-network, send us a Provider Nomination Form, and we’ll contact them directly.2
Continue your current care
If you’re new to Cigna Healthcare and already seeing a doctor that’s not in our network for a specific condition, you can keep seeing them while we help with a safe transfer to an in-network provider. Just fill out a Transition of Care Form within 30 days of starting your new Cigna plan. Have questions? Email us.
