Adding a Provider to the Cigna Healthcare EAP/Behavioral Health Network
When an employee or a Cast Member is in treatment with a provider who is not in the Cigna Healthcare Behavioral Health network, please ask the member to fill out this Provider Nomination form
Confidentiality/Release of information
If there is ever a concern regarding confidentiality or the releasing of information to someone other than the employee or Cast Member, please provide the member with a Release of Information (ROI) form
Claims submissions
To submit a claim for treatment with a behavioral health or EAP provider, an employee or a Cast Member must have their provider fill out a claim form
