If you’re receiving special care, here’s how to continue
If you’re receiving treatment for a specific condition with an out-of-network provider, you can continue until a safe transfer of care is arranged with an in-network provider or facility. Please note, you must apply by filling out the following form and return it no later than 31 days after your effective date.

**Supplemental benefit plan options—Accidental Injury, Critical Illness and Hospital Care Insurance—are available to active associates only. Enrolling in supplemental benefit plan options is limited to Open Enrollment unless you are a new hire or an associate who experiences a midyear qualifying event. You must enroll within 31 days of your effective date or qualifying event.
