Healthy habits can lead to healthy savings

If you complete your biometric wellness screening between November 23, 2024 and November 21, 2025 (11:59 p.m. ET), you will: Save $500 on your 2026 medical payroll contributions. You are no longer required to complete the online wellness assessment to save on next year’s medical payroll contributions.

Schedule your wellness screening at an onsite Screening event or JPMC Health & Wellness Center. Or you can download a form1 to get your screening at a doctor’s office, Quest Patient Service Center/lab, or CVS MinuteClinic.

To schedule your biometric wellness screening:

If your covered spouse or domestic partner also completes the biometric wellness screening by November 21, 2025 (11:59 PM ET), you will save an additional $500 on your 2026 medical payroll contributions.

The first time you access myCigna.com, you’ll need to register and provide your name, date of birth, the JPMorganChase employee’s ZIP code and the employee’s Social Security number.

They say good health is its own reward. It can be even more rewarding when you participate3 in the JPMorganChase Wellness Activities Program.

The money in your MRA is tax-free and can be used to pay for eligible out-of-pocket expenses, such as medical and prescription drug expenses. Earning all available Wellness Rewards in your MRA can really help offset some of these costs.

As a JPMorgan Chase employee, you have the opportunity to earn up to $700 in your MRA by completing wellness activities between January 1, 2025, and December 31, 2025. Visit My Health > My MRA Additional Wellness Activities2 for more details.

1Download and print the Physician wellness screening form and take it with you when you visit your doctor for a routine check-up. Have your doctor complete all required biometric fields, fill in the date of service and sign the form. You must then submit the completed form via fax or upload it to the portal.

Important note: The cost shown on the Benefits Web Center during 2026 Annual Benefits Enrollment already reflects the $500 savings on 2026 medical payroll contributions for you. If covering your spouse/domestic partner, the cost shown reflects the $500 savings for them too. Therefore, if you and/or your covered spouse/domestic partner don’t complete the biometric wellness screening by November 21, 2025 (11:59 PM ET), your payroll contributions will increase in March 2026.
