With this plan, your employer has established a health reimbursement account that you can use to pay for eligible out-of-pocket expenses during the calendar year.

- $3,000/$6,000 employer contribution to health reimbursement account
- Unlimited lifetime maximum benefit
- 100% coverage for in-network preventive screenings
- Plan pays 100% after deductible for in-network inpatient and outpatient covered services
- $3,000/$6,000 plan deductible
- $6,500/$13,000 out-of-pocket annual maximum
- $10 primary care physician (PCP) office visit copay
- $10 specialty care physician office visit copay
- $100 emergency room copay
- $50 urgent care copay
- $10 copay for MDLive by Evernorth® virtual care visit

If you have a medical provider who is not currently in our network, you can nominate them to join the Cigna Healthcare network. Just fill out and send in a Provider Nomination Form and we’ll contact them directly.
